Purpose
• Two-Way Feedback: Both supervisors and employees provide feedback using the “Keep Doing, Do More, Do Less” format, fostering open communication.
• Quarterly Focus Setting: Together, the supervisor and employee identify 1–3 focus areas for the next quarter, creating clarity and accountability.
• Progress Assessment: For all check-ins beyond the first, the form includes an “Assessing Previous Goals” section to evaluate progress toward previously established objectives.
